Faith Formation Information, Schedules, and Forms
Confirmation students will use the regular faith formation registration form, and high school teens who do not require the sacrament of Confirmation will need to fill out the Life Teen interest form instead. Below are all the available forms, schedules, and information for the 2025-2026 Faith Formation year. Please check back for certain Life Teen forms that have not been published yet.
